A 90-minute baseline assessment with photography, range-of-motion measurement, and a written glossary of your specific findings. This becomes the reference document for every visit that follows.

Quarterly or biannual re-assessments that update your folio, redefine any changed terms, and adjust the care plan. Think of it as a revised edition of your clinical reference.

No. Harbor Thread Clinic operates on direct access in our state. If imaging or specialist referral is warranted, we will define why in writing and coordinate the order.
Your folio is a running clinical-reference document we maintain for you. It contains every defined term, margin note, and dated finding from your visits. You receive a printed or digital copy at any time on request.
We organize care around defined terms rather than visit counts. Every session begins by re-reading your current definitions and ends by updating them. The goal is a legible, portable record you understand fully—not a discharge summary written in shorthand.
We are an out-of-network provider. We supply a superbill with CPT codes and defined diagnoses so you can submit for reimbursement. Payment is collected at the time of service.
Frequency is written into your folio as a defined interval—typically weekly during active treatment, then quarterly for maintenance revision. We never prescribe open-ended visit blocks.
